30/20 PLUS
- Pays first $250 of expenses per in-hospital admission and
- $30/day for room and board and $60/day intensive care
- 20% of the next $12,500 of eligible expenses
- Ambulatory Surgical Center benefit
365 PLUS
- Option I: $100 fixed daily benefit
- Option II: $250 fixed daily benefit
- Covers in-hospital stays, convalescent, or skilled nursing facilities
- Ambulatory Surgical Center benefit
STATE INSURANCE SUPPLEMENT (SIS)
- Pays first $100 of eligible expenses per Hospital Admission.
- 10% of the next $25,000 per calendar year
- Ambulatory Surgical Center benefit
