Aflac’s Cancer and Intensive Care plans provide unique coverage for State of Florida Employees that goes beyond standard major medical insurance, paying benefits directly to you regardless of what your health insurance covers. These plans do not coordinate with your major medical plan, ensuring you receive the full benefits outlined in the policy.

Aflac Cancer Indemnity Plan

• Offers cash benefits for cancer-related treatments.
• Provides financial relief regardless of other insurance coverage.
• A previous cancer diagnosis does not automatically exclude you from coverage.

Aflac Personal Hospital Intensive Care Plan

• Pays for ICU admissions, including sickness-related and accident-related admissions.
• Some health-related underwriting questions may apply.

Use Multiple Aflac Policies Together

If you own more than one Aflac policy that covers the same event, you can file claims under both plans and receive benefits from both. For example, if you’re injured in an accident and require ICU care, you can file claims under both your Accident Plan and your ICU Plan to maximize benefits.

Both the Cancer Indemnity Plan and Intensive Care Plan are separate plans requiring separate applications and enrollments through People First.

HOW TO ENROLL

• Eligibility: State of Florida, University, and State College Employees can enroll during Open Enrollment or within 60 days of new employment.
• Enroll: Visit Peoplefirst.myflorida.com or call the People First Service Center at 866.663.4735 and make your election using the plan code associated with your selected plan: 6500, 6501, 6502, 6503, 6510, 6511, 6512, 6513, or 7000. Instructions
• Find an Agent: After you make your election, contact your licensed agent by visiting Capitalins.com/Find-An-Agent or call 800-780-3100 to complete the Enrollment Form. (Pending underwriting review status will be removed upon issue of your policy.)
